At Clinical Consultants, we are dedicated to delivering evidence-based treatment for survivors and offenders of violence. Our specialized services include comprehensive evaluations, personalized individual therapy, and engaging group sessions, all tailored to address domestic violence and provide additional therapeutic supports as needed.

Survivors of domestic violence are not required to bring any evidence or forms of documentation for your assessment or admission into our program. However, it is encouraged that you bring supplemental documents such as police reports or protective orders, so that we are able to better understand your needs and provide you with the most appropriate support and resources tailored to your situation. Your comfort and confidentiality are our top priorities, and we are here to assist you every step of the way on your journey towards healing and independence.

Admissions for Violence Survivors

Before scheduling an assessment, please collect the following information:

  1. A court sentencing agreement should display the case number, the specific charges you were sentenced for, and the detailed orders regarding domestic violence. Moreover, if you are under probation, ensure to obtain a copy of your probation or parole agreement.

  2. To obtain a Police Incident Report related to an arrest or citation involving domestic violence, you need to file a GRAMA Request. You must visit the police station where the responding officers are based and ask for a copy of your incident report. Additionally, sign a release of information form to allow the unredacted report to be sent directly to our business.

  3. To obtain a BCI (Bureau of Criminal Identification) Report, which costs $15, you must request a copy of your legal background to be sent directly to us. For more information, visit the BCI website. Requests can be submitted by mail, which may take up to two weeks for your record to be mailed to you. Alternatively, many choose to visit the BCI office in person for immediate (same-day) processing. Please ensure you visit our office first to collect the Third-Party Release form, enabling the BCI to send your report to us. The BCI office is located at 4315 South 2700 West, Suite 1300, Taylorsville, UT 84129, and can be contacted at (801) 965-4445 for any inquiries.

    *Please be aware that the requirements outlined are mandatory for any provider conducting a domestic violence assessment as per Utah Administrative Code R501-21-7. It is your responsibility to obtain these records; neither your probation officer nor Clinical Consultants can acquire them on your behalf. If you were represented by an attorney in your case, they might have access to these records.
